Part of the history of American Psychiatry

If you want a concise history of the evolution of American (and world) psychiatry over the past
75 years or so, just look at the differences in the "DSM" versions from version I to version
V (just released last year). DSM-III I think of as the beginning of the current period of psychiatry,
with diagnostic categories based less on theoretical models and more on the basis of
inter-rater reliability for the diagnostic criteria, indications of "discreteness" of the diagnosis,
looking at diagnoses from various perspectives (epidemiology, genetics, treatment response,
co-morbidities, long-term course, and other research findings) and, yes, mistakes that get
made, as well. The ongoing problem with psychiatric diagnosis is that there is not yet a
"gold standard" test to definitively pronounce a diagnosis as present or absent; not in the same
way as, for example a chest X-ray and a sputum culture might definitively diagnose a pneumonia.
Diagnoses remain based on clinical profiles, not on clear etiologic bases, so it is like looking at
the state of many medical diagnoses in the late 1800's.
